RESUMO

Los seudoaneurismas de las arterias de la mano son infrecuentes, se han publicado escasos reportes de casos de adultos, secundarios a un trauma cerrado o abierto. El diagnóstico se confirma mediante ecografía Doppler y otros estudios complementarios. Si no se trata, se puede complicar con trombosis, embolia distal de arterias digitales, lesión nerviosa o necrosis. La cirugía es el tratamiento recomendado y consiste en resección del segmento dañado y ligadura simple, arteriorrafia o reconstrucción vascular con injerto. Los objetivos de este reporte son describir un caso de seudoaneurisma hipotenar secundario a una herida penetrante en un niño y, dada la rareza de este cuadro, revisar, de manera no sistemática, la bibliografía actual. Pseudoaneurysms of the arteries of the hand are infrequent; few reports of cases in adult patients, secondary to closed or open trauma, have been found in the literature. The diagnosis is confirmed by Doppler ultrasound and other complementary studies. If it is left untreated, the complications include thrombosis, distal digital artery embolism, nerve injury, or necrosis. Surgery is the recommended treatment and consists of resection of the pseudoaneurysm and simple ligation, arteriorraphy, or vascular reconstruction with graft. The objective of this report is to describe a case of hypothenar pseudoaneurysm due to penetrating injury in a pediatric patient, and, due to the rarity of this condition, to conduct a non-systematic review of the current literature. Resumen: Las acrometástasis se definen como metástasis óseas localizadas distales al codo y la rodilla. Su prevalencia es muy baja, aproximadamente el 0,1% de todas las metástasis óseas y se presentan en pacientes con enfermedad avanzada y son indicador de mal pronóstico. Hasta en el 10% de los casos se presentan como el primer signo de neoplasia oculta. Su forma de presentación clínica y radiológica es inespecífica, lo que genera retraso en su diagnóstico y tratamiento. La resonancia magnética es la imagen de elección para el diagnóstico. El tratamiento en la mayoría de los casos es paliativo. Presentamos cuatro pacientes con acrometástasis y una revisión de la literatura.


Abtract: Acrometastasis are defined as localized bone metastases distal to the elbow and knee. Its prevalence is very low, approximately 0.1% of all bone metastases and they present in patients with advanced disease and are considered indicators of poor prognosis. In up to 10% of cases are the first sign of undiagnosed neoplasia. Its clinical and radiological presentation is non-specific which generates delays in its diagnosis and treatment. Magnetic resonance is the image of choice for diagnosis. The treatment in most cases is palliative. We present four patients with acrometastasis and a review of the literature.

Resumen: Introducción: Los tumores en la mano son poco frecuentes y los tumores malignos son particularmente raros; sin embargo, las lesiones seudotumorales primarias de la mano, como gangliones, son muy frecuentes. La mayoría de los tumores que se encontraron en el hueso y tejido blando pueden presentarse de igual manera en la mano, pero su frecuencia y distribución es distinta. A pesar de la baja incidencia de los tumores benignos y malignos en la mano, éstos pueden ser localmente invasivos o comprometer su función. El objetivo principal de este estudio es determinar la epidemiología, tipos, localización y manejo de los tumores y seudotumores de la mano en adultos en población mexicana. Material y métodos: Se diseñó un estudio retrospectivo mediante revisión de expedientes clínicos de pacientes de nuestra institución con diagnóstico de tumores o seudotumores en la mano de Enero de 2007 a Diciembre de 2016 evaluando variables como edad, género, lateralidad, abordaje diagnóstico, estirpe histológica, localización y tratamiento. Resultados: Nuestro estudio comprende 215 pacientes con una edad promedio de 44.23 años, predominantemente mujeres. Los seudotumores son los que con más frecuencia se detectan (quiste sinovial), seguidos de los tumores benignos de tejido blando, tumores óseos benignos y, por último, los tumores malignos. Discusión: Después de todo, sólo sugerimos una clasificación para la mano que es sencilla y reproducible y dar orientación para el tratamiento. La necesidad de un sistema de clasificación es inminente para estandarizar los diagnósticos, guiar futuras investigaciones, enfocar tratamientos y preservar la función de la mano, independientemente de la epidemiología que se observe.


Abstract: Introduction: Tumors in the hand are rare, and malignant tumors in the hand are peculiar; However, primary pseudotumoral lesions of the hand, such as ganglion, are very frequent. Most of the tumors found in the bone and soft tissue can be presented in the same way in the hand but their frequency and distribution is different. Despite the low incidence of benign and malignant tumors in the hand, these can be locally invasive or compromise the hand function. The main objective of this study is to determine the epidemiology, types, location and management of tumors and pseudotumors of the hand in adults in the Mexican population. Material and methods: We designed a retrospective study by review of clinical records of patients in our institution with diagnosis of tumors or pseudotumors in hand from January 2007 to December 2016 evaluating variables such as age, gender, laterality, Diagnosis, histological lineage, localization and treatment. Results: Our study comprises 215 patients with an average age of 44.23 years, predominantly women. Pseudotumors are the most frequently encountered (synovial cyst), followed by benign soft tissue tumors, benign bone tumors, and, finally, malignant tumors. Discussion: After all we suggests a possible new classification for the hand that is simple, reproducible and give guidance for treatment. The need for a classification system is imminent to standardize diagnostics, guide future research, focus treatments and preserve hand function.

Resumen Las mascotas exóticas, como el erizo de tierra, son capaces de transmitir al ser humano diferentes infecciones, como salmonelosis, micobacterias, protozoos como Cryptosporidium parvum, y dermatofitosis. Presentamos el caso de un paciente adulto masculino, que recientemente había adquirido un erizo de tierra, que presentó en la mano una lesión de tiña incógnita y un granuloma de Majocchi. Se identificó el agente etiológico como Trichophyton erinacei, por cultivo micológico y biología molecular. El paciente se trató con terbinafina por vía oral, por seis meses, con excelente respuesta.


Exotic pets, such as the ground hedgehog, are capable of transmitting to the human being different zoonoses, such as salmonellosis, mycobacteria, protozoa such as Cryptosporidium parvum, and dermatophytosis. We present the case report of a male adult patient, who had recently acquired a ground hedgehog, who presented in his hand a ringworm lesion incognito and a Majocchi granuloma. The etiological agent was identified as Trichophyton erinacei by mycological culture and molecular biology. The patient was treated with terbinafine oral, with excellent response.

El RS3PE es una patología infrecuente de la esfera reumatológica que afecta a adultos mayores, manifestándose como poliartritis simétrica más edema con signos de fóvea. Si bien suele tener una excelente respuesta a corticoides, debe tenerse en considera-ción su asociación a neoplasias, lo cual determina el pronóstico de la enfermedad. Se presenta el caso de un varón de 75 años, con artritis y severo edema de extremida-des, el cual revirtió completamente con el uso de prednisona.


RS3PE is a rare rheumatological syndrome that affects older adults, manifesting as symmetrical polyarthritis and limbs edema. Although it usually has an excellent re-sponse to corticosteroids, its association with neoplasms must be taken into con-sideration, which determines the prognosis of the disease. We report the case of a 75-year-old man with arthritis and severe edema of limbs, which completely reversed with the use of prednisone.

Abstract: Bowen's disease is an in situ squamous cell carcinoma of the skin with only 2% of pigmented cases reported. It is clinically characterized by papules and plaques of blackened surface that may be caused either by sun damage - usually in photoexposed areas in elderly individuals - or by human papillomavirus infection - usually in the anogenital region of young adults. Dermoscopic aspects of Bowen's disease are discussed for over a decade, but with no definitive criteria that would lead to a definitive diagnosis. We present a case of Bowen's disease affecting the finger of a 57-year-old Asian patient. The lesion clinically and dermoscopically simulated a melanoma. Histopathological findings suggested the diagnosis of pigmented Bowen's disease. Pigmented Bowen's disease should be considered a differential diagnosis of melanoma, since its clinical and dermoscopic criteria are unspecific. Histopathological examination remains the gold standard for the diagnosis of the disease.

RESUMO

Abstract Bier spots are small, irregular, hypopigmented macules that are usually found on the arms and legs. The macules disappear when the limb is raised. Bier spots have been reported in association with a number of conditions but there is no consistent association to specific desease. Although they usually affect young adults, we report a case of Bier spots that began in childhood. As an asymptomatic and possibly transitional condition, the disease does not require treatment.

Introdução: A sindactilia é uma anomalia congênita comum da mão, tem incidência de aproximadamente um em 2.000 nascimentos, predomínio no sexo masculino e é bilateral em 50% dos casos. Objetivo é descrever uma série de casos de pacientes com sindactilia nas mãos, operados no Hospital Sarah Brasília, com a técnica de Skoog modificada e o uso de órtese. Métodos: Realizou-se a técnica de Skoog modificada para correção de sindactilia. No pós-operatório, foi confeccionada órtese punho-mão e dedos, e procedeu-se à avaliação do resultado da cicatriz conforme a Escala de Vancouver. Resultado: Durante cinco anos, avaliaram-se 45 pacientes, sendo 30 (66%) homens e 15 mulheres, com idade de um a quatro anos (média de dois anos - 60%), 37% dos casos mostraram-se associados a síndromes, 50% eram simples e bilateral. As complicações observadas nas sindactilias simples foram encontradas em 8% dos pacientes e caracterizaram-se por retração cicatricial, cicatriz visível e alteração ungueal; três casos (6%) foram reoperados para liberação de retração cicatricial com zetaplastia e confecção de neocomissura e quatro casos demandaram realinhamento digital, observado nas sindactilias complexas. Conclusão: Com esta técnica, obtiveram-se 92% de resultados funcionais, 8% de complicações relacionadas à técnica cirúrgica e três reoperações para liberação de retração cicatricial, por meio de zetaplastia e aprofundamento da comissura com novo retalho e enxerto de pele Em relação à avaliação quanto à Escala de Vancouver, o item predominante foi a pigmentação existente entre os dedos devido ao enxerto de pele.


Introduction: Syndactyly is a congenital anomaly of the hand, occurring in approximately 1 in every 2000 births. It is more common in male patients and is present in both hands in 50% of the cases. The objective of this study was to describe a series of cases of patients with hand syndactyly, operated at the Sarah Hospital in Brasília using modified Skoog's technique and orthosis. Methods: The modified Skoog's technique was performed to correct syndactyly. Wrist-hand and finger orthosis was applied postoperatively, and the appearance of the scar was assessed according to the Vancouver Scar Scale. Results: Forty-five patients were assessed during five years, of whom 30 (66%) were male and 15 were female, all aged between 1 and 4 years (mean age: 2 years [60%]); 37% of cases were associated with syndromes and 50% were simple bilateral cases. Complications observed in simple syndactyly occurred in 8% of patients and included scar retraction, visible scars, and ungual changes; three patients (6%) underwent revision surgery with z-plasty and confection of a new web space commissure to correct scar retraction, and four patients required finger realignment, observed in complex syndactyly. Conclusion: This technique resulted in 92% good functional outcomes, 8% complications associated with the surgical technique, and three revision procedures with z-plasty and confection of the web space commissure with a new flap and skin graft to correct scar retraction. The Vancouver scale was used to assess scar quality, and pigmentation between the fingers (due to skin graft) had the highest score.

Lipoid proteinosis is a rare autosomal recessive disorder of variable severity that may involve the skin, mucous membranes of the upper respiratory tract and internal organs that may display different clinical manifestations based on the site of involvement. A 34-year-old male patient was admitted to our hospital with a complaint of hoarseness present since childhood but had worsened in recent years. A dermatological examination revealed light brown papillomatous skin lesions on the dorsum of both hands and elbows present for a long duration. Endoscopic examination of the larynx revealed widespread mucosal thickening and irregularity. Microscopic examination revealed acid Schiff (PAS) positive, Congo red negative eosinophilic hyaline material deposits in the dermis and submucosal regions. The final diagnosis was lipoid proteinosis with skin and larynx involvement. This typical case has been reported to draw attention to the fact that lipoid proteinosis should be included in the differential diagnosis of patients presenting with hoarseness in childhood.

OBJECTIVE: To assess the feasibility of visualizing hand and foot tendon anatomy and disorders by Gemstone Spectral Imaging (GSI) high-definition CT (HDCT). MATERIALS AND METHODS: Thirty-five patients who suffered from hand or foot pain were scanned with GSI mode HDCT and MRI. Spectrum analysis was used to select the monochromatic images that provide the optimal contrast-to-noise ratio (CNR) for tendons. The image quality at the best selected monochromatic level and the conventional polychromatic images were compared. Tendon anatomy and disease were also analyzed at GSI and MRI. RESULTS: The monochromatic images at about 65 keV (mean 65.09 +/- 2.98) provided the optimal CNR for hand and foot tendons. The image quality at the optimal selected monochromatic level was superior to conventional polychromatic images (p = 0.005, p 0.05), compression (chi2 = 0.5, p > 0.05), absence (chi2 = 0, p > 0.05) and rupture (chi2 = 0, p > 0.05). GSI was significantly less sensitive than MRI in displaying tendon adhesion (chi2 = 4.17, p < 0.05), degeneration (chi2 = 4.17, p < 0.05), and tendinous sheath disease (chi2 = 10.08, p < 0.05). CONCLUSION: GSI with monochromatic images at 65 keV displays clearly the most hand and foot tendon anatomy and disorders with image quality improved, as compared with conventional polychromatic images. It may be used solely or combined with MRI in clinical work, depending on individual patient disease condition.
